Here are some forms and related resources you will find useful as a member of the Pack160 Family. Many of the forms are needed if you or your den are planning an outing or other event.

Pack Expense Report

You'll need this form if you purchase supplies or food for a Pack event. The form needs to be submitted with receipts for reimbursement by our treasurer.

Application to Join Cub Scouts

You'll need this form for each new Boy that joins the pack. It is filed with the council so that each boy is registered with the national Boy Scouts of America.

Medical Form

This form is needed for adults or boys participating in summer camp and other council run overnight programs.

Adult Leader Application

This form is needed for all adult leaders participating in the pack. It is filed with the council so that all adult leaders are registered with the national Boy Scouts of America.

Local Tour Permit

You'll need this form if you are coordinating an outing for your Den or the Pack. It must be filed with the local Monmouth Council office prior to the outing.
